Use the chart below to determine what information applies to you. Click on the appropriate link to view and/or print each form you are required to complete. The columns are defined as:
  • Preschool = Preschool and PreKindergarten
  • New K-12 = New families who wish to enroll their children aged Kindergarten and older
  • Returning K-12 = Families who had K-12 children enrolled the previous school year
  • Homeschool Class = A home schooled student who wishes to enroll in one or more core classes
  • Homeschool Extra = Home schooled students who wish to participate in athletics, band, or choir.

    Preschool and PreKindergarten Information and Application Children must be 4 years old by September 1. Admission requires interview with Administrator. Yes        
    Application for K-12 For new families who wish to apply for admission for their child(ren) in K-12. Child must be 5 years old by September 1 to enter Kindergarten. Admission requires interview by Administrator and at least one Board member.   Yes      
    Pastor's Recommendation This is part of the application process for new families.   Yes      
    K-12 Enrollment For ALL K-12 students, new (after successful application for admission), and returning.   Yes Yes Yes  
    Enrollment form for homeschoolers For homeschool students who wish to participate in band, choir, or sports other than varsity sports (those wishing to participate in varsity sports need to enroll full-time).         Yes
    Field Trip/Medical Release Required annually for each student. There is room on the form for up to 8 children. Yes Yes Yes Yes Yes
    Parent-Guardian Agreement Required annually for each family.   Yes Yes Yes Yes

    Permission for Personal Laptop Only required for secondary students (grades 7-12) who wish to bring their own laptop computer to school. Form must be submitted annually. Use a separate form for each student.   Yes Yes Yes  
    Photo Release Required annually for each family who wishes to give us permission to use photos on the web site and/or in newspapers. There is space on the form for 2 adults and 10 children. Yes Yes Yes Yes Yes
